Home Health care



Our Mission is to provide the highest quality care to our patients who are elderly, disabled, or transitional. As an Agency, we will promote living with dignity for all our patients by maintaining the highest quality care standards, and by anticipating and reacting to the individual needs of each patient.​ We promise that we will be there when you need us, as we staff on-call skilled caregivers 24 hours a day, 7 days a week.

We are proud to offer

Skilled Nursing:

Skilled nursing is provided by our experienced, highly skilled, and compassionate nursing team under the direction of your physician. Comprised of RNs and LPNs, our nurses coordinate each patient’s care based on their individual needs, and are on-call 24 hours a day for consultation. In addition, they educate patients and their families about treatment options, ensure medication education and compliance, perform wound care, manage IV therapy, disease management, perform health status assessments, and much more.

Physical, Occupational & Speech Therapists:

Medical services provided by a licensed therapist, or licensed therapy assistant under the direction of the therapist. Services are coordinated by our Agency as recommended by your physician. Therapy services can help you recover from an injury, manage a disability, and help improve communication skills that are affected by a medical condition.

Medical Social Workers:

Services may include short-term counseling services, referral to and coordination with community resources and assistance with living arrangements, finances, and long-term planning.

Home Health Aides:

Services are delivered under the supervision of a licensed nurse or therapist. Part-time assistance with personal care is provided when added to your plan of care. Typical duties include bathing, changing bed linens, and assisting with other activities of daily living.

We help also help provide clients with services in the comfort of their own home, wherever that may be, which includes the following:

Wound Care Nurse Treatment:

Our wound care nurses specialize in assessing, treating, and managing various types of wounds, including chronic and acute wounds, surgical incisions, and ulcers, focusing on preventing infections and promoting healing.

  • Ensure patient comfort
  • Monitor healing
  • Reduce readmission to the hospital for infection and complications
  • Prevent future wounds and educate

LSVT BIG Parkinson’s Disease Therapy:

This program helps patients with Parkinson’s Disease (PD) reclaim their movements and independence. Our physical therapists have special LSVT training to help you or a loved one with everyday tasks. 

  • LSVT BIG is a powerful therapy that trains you to use your body more normally, combating the smaller and slower movements often seen in PD.
  • Many people with PD face challenges with daily activities, whether it’s buttoning a shirt or getting up from a chair. LSVT BIG effectively recalibrates how you perceive your movements, teaching you how to apply extra effort for bigger, and more natural motions. This leads to improved walking and balance, enhanced self-care and performing daily tasks with more ease. 
  • Whether you’re in the early stages of PD or facing more significant challenges, LSVT BIG can make a meaningful difference. Starting therapy early can lead to the best results, but it’s never too late to begin your journey toward improved mobility!

Cardiac Rehabilitation Program:

A program designed to improve an individual’s overall health and well-being, while teaching heart-healthy habits.

  • We provide an ECG Machine, perform a tracing after exercise, and send the results to your doctor.
  • Medication education
  • Help with adapting to a heart-healthy lifestyle, including exercise routines with low impact.

Anodyne Infrared Therapy:

An innovative non-invasive, painless therapy that uses infrared light for the treatment of various medical conditions.

  • Reduces chronic pain and decreases muscle spasm activity.
  • Increases blood circulation and accelerates wound healing.
  • Treatment is non-invasive and has virtually no side effects.
  • Enhances natural healing mechanisms within the body.
